My current plan is:

1. Do a virtual consultation w/ Giotikas like April of 2022 ish , gather the labwork/x rays, etc needed to clear me for the surgery, etc

2. September of 2022, Giotikas femurs to 7.5 cm w/ weight bearing Gnail

Then wait a year to recover

3. 4 months before planned surgery, do a virtual consultation w/ Parihar + start gathering all the labwork/xrays etc

4. Then to save money, go w/ Parihar 6 cm tibia Precise 2

And then my hope is w/ the experience of doing it once w/ Gnail, I'll be ready for a more difficult challenge of:

- not being in a western nation
- wheelchair the whole time w/ Precise 2
- differences in cleanliness of Greece vs India
- I'm guessing that Athens, Greece and Mumbai have similar percentage of English speakers?

I am wondering to myself 'is my idea retarded? Am I overlooking anything?' I emailed Parihar asking if he uses gnail. He just said look at my website and I only see mention of precise 2. Is my timeline not realistic/not enough time to do things like get the labwork/prepare?

Who told you that lol? There is no country where it won't be considered a crime. A doctor can't reveal anything at all. We do have privacy laws. India is just another country where humans live lol.

total cost of precice 2,2 in india -> 32,844 USD
Surgery cost -> 11,822 USD

Giotikas price -> 46,841.40 USD

so parihar is still cheaper but the difference is like $ 2k.

Reaching Lee is very easy while Parihar very Hard , If You have any problem Lee Institute will listen while with Parihar you have to take appointment which may take days

Yes parihar is very hard to get hold of for a consultation or even to get an email reply. But once you become a patient they will respond well.

For a USA citizen, on the east coast, what are the pros and cons of Dr. Parihar vs Dr. Giotikas?

I think that the price of travel vaccines + extra price of the ticket cost pretty much 2000 USD which makes me think their prices is now the same

Does Dr. Parihar do rebreaks of bones?

Ex: what if I lengthened just 1-2cm on tibias with another doctor (considering doing gnail 7cm on femur then 2 cm on tibia with precise 2 back to back with only 1 month rest in between, might be enough lengthening tbh but want the option to rebreak the tibias later if want more length). Purpose of back to back: get that precise 2 nail before prices increase from inflation

Or at that point, would it be cheaper just for me to do rebreak of the tibia bones in the USA with a local leg lengthening doctor? (there are two who are within a few hour s driving distance of my place)?

He's one of the Indian docs I think people can be sure isn't a quack. He's trained under Dr. Paley.

for a year only. if there was a recent HSS affiliation at some peer reviewed case study perhaps this statement would hold some weight at assessing the pearls of Mangal's surgical techniques. I'd project that at 2022 it takes around 15 years at an elite community to refine surgical technique to become an expert and approximately 30 without these resources. You would keep this in mind while selecting a surgeon in general

Im interested in LON method on tibia but think im not the right candidate and also ive heard there is some risks to have prolonged knee pain.

Anyways from what i understand in my case the right option is to have external fixator that, from what ive read in the past, has to be on your legs for the whole process until you can walk fully.

But recently i saw in a interview with indian surgeon Dr. Mangal Parihar he said that they now can use "locked plates" that they can do the lengthening with a fixator but then put a plate on so that the patient doesnt have to have the fixator for a long time.

Does anybody have experince/knowledge about this? Thanks.

It's an alternative to putting in a nail to decrease the time in an external fixator. As I understand it, it's not as stable as LON and results in more incisions being made, but I may be wrong. Dr Birkholtz offers that as an option too, last I checked.

Stumbled upon this old post on the forum. Attached below, we have straight from the godfather of modern limb lengthening, Dr Dror Paley, that Dr Parihar is a very good doctor. And Paley doesn't blindly support all docs who have studied with him or at one of his facilities, as we've seen him trash a doc associated with him before.

(The part about Stryde didn't age well though lol)

1.do they provide you with physiotherapy during the lengthening phase or distraction phase?

2.since you increased by 6 cm, how long did your lengthening phase last? and if I recall correctly, you didn't have an intramedullary nail in place, so you had to wear it during the consolidation as well. If I opt for LON external fixator, how long will the lengthening phase last and probably I can return home

3. Can I expect assisted walk after say like 90-100 days of lengthening? Yeah I've got some budget constraints in placed as well distance issues so I'd like to return home and also avoid complications, if any are to arise, and if embolism is something, then I'll probably not return until I get unassisted walking, how long does it take for unassisted walking over LON external fixator?

4. How can I get more information about this from you? Like some more details, I'm considering it getting done sometime in the future, and have you regained most of your athletic abilities now? I'll make up my mind based on that and probably contact doctor Parihar

1) He offers externals, externals with hexapod, and LON. Looking to offer internals again once Fitbone becomes available.

2) He doesn't offer comprehensive packages that include stay, as short term rentals are hard to come by in Mumbai. Best advice would be to work out a deal with a nearby hotel. Many of them have significantly reduced rates for stays of a few months, and I got one that worked out to $50 daily. Medications during the hospital stay are included, but anything additional after that is out of pocket. He works with a guy who is able to provide walkers, wheelchair, etc, but that is an out of pocket expense. There are very budget friendly options and more expensive options depending on your needs.

1) Physiotherapy is included as part of what you pay for the surgical fees, while you're in the hospital. After you leave, you have to pay for physiotherapist visits, depending on their individual rate, which is partly based on how far they have to travel to get to you. I think my physiotherapy averaged out to $11 per session at the hotel. Any physiotherapy I received at Dr Parihar's clinic during his biweekly checkups ended up being free.

2) I was there from the end of June until the beginning of October. Lengthening began one week after the initial surgery. If you go the LON route, the fixator is removed once your lengthening is over. You just have to stay in Mumbai during the lengthening phase, and when you return home you just send x-rays to Dr Parihar once a month so he can check on your bone healing.

3) LON allows early removal of the fixator, but you won't be allowed to graduate to crutches/walker until Dr Parihar is satisfied with your level of bone healing to allow it. How early depends on individual genetics and stuff.

They will appoint someone to take care of your needs while you're in the hospital. That'll include things like giving you your food and emptying your pee jug and all that. Once you're out of the hospital, that's up to you to arrange. It's one of the reasons I recommend a hotel, as the staff will take care of things like bed sheet changes, emptying trash, getting you transportation to the hospital, etc. You can leave after the lengthening phase and the fixator is removed, in case of LON. For 6 cm, probably 3-4 months.

1) They've always had LON. Physiotherapy is provided at no charge while in the hospital, or when you come in for your checkups. They give you a worksheet of exercises to perform on your own, and if you elect to have more physiotherapy sessions at your place of stay, they can arrange it for you, but it's at your expense. It's cheap though. He will expect you to continue your exercises when you are done lengthening and still in consolidation, and will request follow-up x-rays once a month so he can check on your bone healing.

5) I stayed at Hotel Maharana Inn a few blocks away from Dr Parihar's clinic: https://www.hotelmaharana.com/ I went to India by myself. Anything I needed while at the hotel was taken care of by the staff that worked there.

6) LON has a risk of chronic knee pain that's referenced in the medical literature. Some doctors say the rate is 20-25%, but so far Dr Parihar hasn't had that come up in any of his patients.

Thank you so much for this, kilokahn, thanks for providing me with all the info you observed on ground, ahh one more thing, though doctor Parihar didn't have any case were the patient developed a knee pain, just to be on the safe side, considering the external fixator without the nail, sounds just the thing for me, but my only problem is the prolonged stay, in your case, as you mentioned that you had to stay there from the end of june to the beginning of october, was it just from the surgery till the end of lengthening, or did it also include consolidation? How long did you wear the frame for? Your stay at the hospital was a week long? Or was it a bigger time period? Thanks a lot kilokahn, for even providing me with the link for the hotel, and I'm very sorry for asking so many questions
