Pain after surgery
« on: November 10, 2021, 02:49:37 PM »

Hi everyone !
I am a prospective LL patient willing to do max 6cm on femurs via internal lengthening .
This Is a topic covered many other times but there have been probably advancements in pain meds/protocols since many years ago .
A lot of past diaries seems to agree that first 3 weeks are hell on heart while after it settles and when lengthening is stopped pain goes away .
They also say that first 2 or 3 says are unbearable.
Most recent diaries I read they talk about max 3/10 pain first days but sometimes even just 1/10 .
After, they have oxycodone and tylenol and pain seems to be under control within the 5/10 range .
It is a huge difference and although I understand that every person have it's own pain tolerance and body ,overall diares have general points in common .
I wonder if there has been an advancement on pain medications or the full weight bearing nails kinda cut down the pain ?
Protocols ?
What are your guys thoughts ? ( I am talking about femur lengthening post-op to 3 weeks post OP not about the pain due to muscle tightening and so on ) .

It depends where you go to do your CLL.

In turkey, for me, the first two days are a 10/10 in terms of pain. It drops to 8/10 for the next two weeks then all the way down to a 4-2 for the 2-5cm stretch, IF you don't get infections. This is for LON, this is my experience. Some patients can handle this bravely - I cannot. Be honest with yourself in terms of how you manage pain. I thought I was a big tough dude (I wasn't LOL).

My thoughts are between Dr.Lee,Dr.Rozbruch and Dr.Assayag .
I don't evaluate any other doctor at this point .

Given that they all are good doctors performing in great Hospitals/institutes I wonder if that can make a difference in term of pain management.
I don't feel I have a big pain tolerance because I never experienced bad fractures or traumas (thankfully)  ,So having pain under control might be key not only for my physical well being ,but for the mental hit post-OP.
Dealing with the the fear, the money spent, the stress ,the pain and then (at least) temporary regret in a moment where you are mentally and physically weak might really have a HUGE hit , no matter how bad you wanted the height .
So the pain control I feel is key for the mental health post-OP and once you have gone trough those weeks with pain under control I feel you are good to go if under the right hands .
Given that I don't aim to 7 or 8 cm ,not even full 6 cm tbh , stretching will probably hit me last days of lengthening so that doesen't concern me much .

If you want to be more comfortable during lengthening, i think Donghoon is the best choice as you can stay in the hospital during the whole lengthening. So in case you feel too much pain at night or can't sleep you can ask nurse for help anytime.

I feel so too .
From what I read I also saw that Dr.Lee keeps patients 1 week and not 3 days post op under supervision which might be draining due to lack of privacy or just general personal time but might be wsy safer as well since I always believed that 2 or 3 days post op are really not enough to send someone home .
So ye Lee first choiche but he has to be more responsive .

I did femur lengthening with the Precice 2.2 internal nails last March. For internal nails, patients typically report pain from 1 to 5 (out of 10) for the first few days after surgery. Around 3 to 4 is most common (like the top of a bell curve). Max short term spike pain levels rarely pass 6. This seems consistent with my experience and other internal nail lengtheners I've talked with or read their diaries.

For external fixators (like LON), I only have information on what other LON patients have stated on their diary or videos. It is obviously more painful than internal nails. External tibia lengthening users usually report pain of 7 to 10. External femur lengthening is even more painful (like 10+) because the upper legs have much more muscle and flesh, and is not recommended. However, the tradeoff is that LON is considerably less expensive than internal nails. So you get what you pay for, "price vs pain".

Thank you for your detailed answear .
I generally read that 2 days after surgery is almost painless because of meds post anesthesia IV and so on .
After it becomes uncomfortable but as you stated nothing crazy and after distraction It kinda disappeares.
What I am concerned about is that some people report 8-9/10 pain with internals too and they compare that pain to crazy things .
What I am worried about is that I don't know what to expect since pain is pain and when you don't have it you think you can handle everything but then when it hits ...so I am trying to have as many PRECICE pain experience I can so that I will know what to expect .
I still didn't figure out if the patient or meds protocol makes a difference in all this ( and if yes ,by how much) .
You went with Dr.Assayag and he is one of my options so glad to hear you did good .
