A few updates

Hey everyone just wanted to drop by with a few announcements:

1) This Thurs 9-30 I will be having Liam back on for an update who did quadrilateral lengthening last October gaining 12.4cm (7 something in femur and 5 something in tibia) via precice 2 nail. He has lots of content to share (videos of physical therapy, walking gait improvements, pictures of before and after etc.) Only thing is he lives in Germany and so I’m planning to do the LL LIVE on my break midday for about an hour around 1pm EDT or so (I’ll lock in time when he confirms) but feel free to post questions below and I'll drop in to get them beforehand or send them to live@cyborg4life.com. I'll actually try to post the link to it on this thread afterwards since I know a lot of people ask about two lengthenings. Let's just say I'm rather impressed with his progress in under a year.

2) Then, the following Thursday 10-7 at 6:30pm EDT, we have a fan-favorite surgeon returning to the show for LIVE Q&A. That’s right Dr. Michael Assayag from ICLL via Sinai Hospital in Baltimore will be coming back to make sure your questions and concerns are resolved. Same thing, post questions in this thread or send email as above.

3) Finally, a person reached out to me and offered to create a d server which I guess is kind of like a fast response chat software. This was a suggestion I got but I'm not super techy so this is super cool of him to do this for the community so if you’re interested in having another place to hangout and chat LL you can join. I think this is a also a good way you can reach out to me with video requests or potential collaborations etc.

Hi, I have watched your videos on YouTube. I enjoyed them. I am so happy that you are also participating on this forum. Limb lenghening really is a surgery that changes some people's lives, helping them live better and happier. I really like your channel.

I just watched episode 11 where you interviewed Tom for the surgery, it is awesome. I would love to join live chat on your channel the next time, so I could ask question too. I am very happy for Tom, he looks so happy now. I want to give my feedback to the video. During the interview, what is the most attractive height in the US has been talked about. When Tom said 5'8, I was amazed! I guess that makes lots of men happy now. Is Tom also a registered user on this forum. Tom I like the way you presented your answers, well done!

Thanks for all the amazing and informative content you put up on Youtube! If it wasn't because of you, I wouldn't have been able to make sense of such a controversial surgery and make the best choices accordingly. I reckon you're going to have an interview with Dr.Assyag this Thursday. Good luck with that! :) Anyways, I have a couple of questions for Dr.Assyag if you don't mind asking him... Here they are:

1- How likely is it to develop Chronic exertional compartment syndrome (CECS) after tibial lengthening of between 5 and 6 cm? Are there ways to prevent this from happening, and if someone were to develop such a condition, Is fasciotomy a must?

2- One of the possible long-term complications of tibial lengthening is the fibular migration in the ankle joint that can lead to arthritis even if the migration amount is small. To mitigate this, applying the proper technique when inserting the syndesmotic screw is crucial. Namely, a quadricortial positioning of the screw leads to lesser migration compared to a tricortial one. My question is does it really make a difference if a surgeon preferred to use a tricortial syndesmotic screwing over the other one in terms of avoiding complications? 

3- Some surgeons such as Dr.Giotakis do not insert a syndesmotic screw proximally, as they believe it's not needed. what's your take on this?

4- I am considering lengthening my tibias by 5 to 5.5 cm. But I have flexible flat feet and my legs are slightly bowed. I am worried that if my bow legs get fixed, I'd have trouble walking and maintaining my balance due to my flat foot. Should the femoral-tibial angle remain the same in my case or not?

Hi Cyborg can you also ask about release surgeries for femurs.

Good question, but I can probably answer that better as a patient who had major ITB issues.

You need to release the ITB because its a fiberous tissue. It is unstretchable. It's your TFL, a relative small muscle that mainly does the stretching. It is very difficult for it to stretch the entire 8-10cm alone.

Hamstrings and quads are actual soft tissues. Severing or loosening their integrity could lead to long-term consequences. Plus, since they're large muscle groups, they can easily handle a 8-10cm stretch.

Yeah I know right great questions chasingthedream! Can't wait to have you back on the show Dr. A.

And Highest I'll be sure to ask about the other prophylactic releases surgeons might use but SirStretchAlot (love that name lol) is right, the TFL or tensor fascia latae sort of merges into the IT band which is very thick and tough tendon which doesn't play nice to stretching. So besides the intra-op release, using certain abductor rotational stretches are about all you can do for relieving stress on it.
