I have surgery (Tibial LON) scheduled for next week, so with all set and done now I can't bother but be a little nervous about the pain right after surgery, which is what people say hurts the most.

That considered my doctor said he always uses Buprenorphine Transdermal Patch, Metamizole (dipyrone) and Tramadol if need.
I've never heard about Buprenorphine but apparently, it is 100x stronger than morphine, and I haven't heard these patches being discussed here either.
Anybody with experiences related to this?

Also for the veterans, what painkillers were you given by your doctor and how would you rate the pain right after surgery?

I'm going with spinal anesthesia also, just wondering about the pain on the first days after surgery since I've heard it's the most painful, but there are also many places that dong give that many painkillers.

Not a veteran, but I was given fentanyl for the first three days after the surgery, of which I could self administer up to 20 tablets per day. Like everyone else I experienced constipation and a heavy breath. Make sure you have no breathing / lung conditions before you take opiates and that your blood pressure is normal.

After that I was prescribed Tramadol as well as Novalgin (Metamizole). I have discontinued the latter as it can lead to agranulocytosis, and is banned in the UK (as well as US and other countries) for this very reason.

Of all the pain meds I find the Tramadol the easiest to tolerate. I try to protect my stomach by having two glasses of Kefir per day plus the Pantoprazole Betz prescribed. My personal feeling is that the Kefir did help my stomach calm down.

The worst pain likely was at the beginning on day one after the surgery. At that point simple things like bending my legs was painful. My legs were heavy and sore. When I walked I could feel all sorts of aches around the incision sites. At that point the opiates helped me get through the day as well as get some sleep at night. I used about half of my allowance.

After a few days the surgery pains will begin to get better until they disappear at around week four to five. At the same time, the lengthening will increase the stiffness in your legs and may cause different kinds of pains such as nerve pain. So it's a bit like one kind of pain being gradually replaced by another kind of pain. It does suck.

Luckily, I did not have nerve pain so far so can't comment on it in any more detail. The stiffness, i.e. stretching pain, starts out very moderately at around 1.5cm until it becomes much more noticeable and annoying around 4 to 4.5cm. So far the Tramadol is still enough and I definitely don't want to take this beyond a point where it would not be anymore.

I had fentanyl for first four days after surgery along with paracetamol (via IV). Once discharged from the hospital, I was on Tramadol, Lyrica and Paracetamol until lengthening was finished. Personally, I didn't find tramadol to be very effective after the first couple of weeks but kept taking it anyway per doctor's orders. Paracetamol was my main line of defense for most things related to pain.
