Dr. Paley or Dr. Guichet?
« on: April 25, 2014, 07:05:04 PM »

I do not mind going in either of them. I just need people's opinions on who is more worth it or if they are both good enough so going on either of them won't change anything.

If you have the money I would go to Dr. Paley without even thinking. Not only is he extremely experienced and a pioneer in the field limb lengthening (both cosmetic and reconstruction) but he is also very personable and good with people. Dr. Guichet is not as personable, he is very serious about the best outcome for his patients and he is a good doctor as well so in the end it is up to you.

Both are highly respected and have a great degree of experience. I wouldn't mind going to either of them (if I had that kind of money). They do seem to have very opposite ideas about some things though. Dr Guichet stresses the importance of exercise and makes you do training at the Isokinetic Center before your surgery, which some people really aren't keen on doing. Dr Paley is of the opinion that Dr Guichet's required training is a bunch of nonsense and has nothing to do with how much you can lengthen. Dr Paley says that you need to have an ITB release done for femur lengthening if you want to ever walk normally again, but I haven't heard of Dr Guichet doing ITB releases. That makes me wonder if the releases really are required to be able to walk normally later or if they're done because the PRECICE won't lengthen properly without it.

I have to agree with Paley that Guichet's training regimen is a bunch of nonsense.  Why?  Because every single other limb lengthening doctor does not require it at all and there is zero evidence to suggest Guichet's patients recover better or faster.  If this doesn't imply that his regimen is a waste of time, I don't know what does.

Did internal femurs using the PRECICE2 with Dr. Donghoon Lee in South Korea on December 27th, 2013, went from 5'7.5" to 5'9.6".  Will probably end up doing tibias in about a year with Dr. Birkholtz to get to 5'11".

I think the precice 2 is so fragile, did you see some of the FDA device malfunctions?

I would like to know if there is any documentation of how much weight the Gnail or betzbone can hold per leg, atleast we know that precice can only hold 75 lbs per leg.

Not to mention a huge waste of money.

I think it really just comes down to your preference for nails when deciding between the two doctors. Do you want to be clicking and riding a bike the day after surgery or do you want to be using ERC devices while staying a lot less active?  I chose Paley because he has tons of experience and PRECICE appeals to me more than the gnail does.

Guichet gets good results too. He is against ITB releases. He encourages exercise before LL.
Why do something unnatural(ITB release) instead of something natural(exercise) to get the same results?

And GNail has been consistently the same for many years. Precice has been evolving and hasn't stabilized yet.

LL itself isn't natural. You're lengthening your body in one area, why not relieve tension in soft tissue when you can?  Also, it's not proven you can have same results, with or without ITB releases.

I'm pretty sure Dr. Guichet is the same way.  Ie his views on pre-OP training and ITB releases, he talks very assuredly of himself and dismisses the opinions of other doctors.

Thank you guys for your answers, though I am wondering how is Guichet's training nonsense? Exercising is good for you so I don't get how that is bad. I agree that he is the only doctor that recommends this but maybe there is a reason.
Generally Dr. Guichet suits me better because he is closer to my country and it is less costly for the travelling. Not to mention I do not have to pay in dollars and maybe lose money in the exchange rate.

I think after the things I have read about Guichet lately I have to put him near the top of my list.  Yeah I don't get why they say his training is nonsense.  I think everyone can agree that the better shape you are in and the more flexible you are will help you with lengthening.  I think his requirements may be extreme but he seems like a perfectionist and he really wants the best outcome for each patient.  Much better than an Indian doctor who just says "don't worry, it will be ok".

Dr. Guichet all the way!

When I first heard about ll I thought "wow, you would really need to be flexible to have this kind of operation!"
So, since he's the only doctor demanding this, he's the only doctor that makes sense to me!

All the others doctors say: "yeah you can barely touch your toes, no problem! we'll stretch your limb even more without any preparation whatsoever!  It does work anyhow but I prefer to be well prepared and have better chances of recovery.

I'm wondering about this myself. I've seen what I think is the most impressive recovery in ShyShy, and the costs are more attractive.

I am pretty fit and flexible, and would prefer being pushed and having a good recovery than not being pushed and having a mediocre one.

Paleys main perks are being in the US and his impressive number of surgeries.
