I've watched the vid in its entirety now.

I just wanted to make some comments:

• At 36:40, they got the Imperial and metric measurements at drastically different levels. 5'6" should be about 167.7cm, and 142cm is about exactly 4'8".
• Just a little anecdote of mine, but the 50yo+ patient who returned to rock climbing, skiing, etc, maintained a good tibial-femoral ratio, and didn't lengthen more than 5cm in either segment, initially.
• Smokers are an undeniable risk group for LL. Paley flat out won't accept them. He didn't even want to add comments like "maybe it's fine if you quit it 3 months before the surgery". I'm guessing he just doesn't want to deal with the risks.
• Estimated 4% chance of fat embolism. Paley mentioned a sample size of 100. All cases happened in bilateral femoral lengthening.
• All complications involving Precice nails and CLL supposedly resolved. The longest complication (Peroneal Nerve Injury) took a year to resolve.
• Of those complications, one was Deep Vein Thrombosis (DVT). Paley mentioned all patients are on DVT medicine from discharge, and stay on it until they fully weightbear ("5 months on medication").

More passing comments: I know Paley said most of his patients think they have short legs for their total size, and that they actually don't, but I'll be damned if that man in my first timestamp didn't look all torso.

If someone lazy is reading this, but still planning to undergo CLL one day, you should still watch the presentation in its entirety. Paley mentions a lot of very important information regarding flexibility and stretching tests, proportion ratios, down to how he performs his surgeries and lessons learned from bad cases of his past. Also, general information about bone breaks and PT.

Good outline, definitely a lot of good nuggets in this video.

To add to the smoking bit, it's just unrealistic to think that a smoker will quit for CLL. The pain, frustration, and long hours of boredom will surely break most smokers. It's not a risk that many doctors would like to take on, since it might taint their track record.

My takeaway is that it's not a necessity for relatively simple manipulation like cosmetic limb lengthening (and definitely doesn't hurt to have it), but it's extremely helpful for complex deformities.

You seem to be extremely knowledgeable on this procedure. Are you aware where the field is heading from here, do you know of any upcoming advancements?

It seems like the internal nail and then the full weight bearing Stryde were two major ones that happened recently but while trying to learn about and consider this procedure I think it's be a good idea to know what could be available soon.

Another amazing interview Cyborg, great job!

I was really impressed with Dr. Debiparshad. I loved his mindset -

• Not all patients suffer from height dysphoria / some form of a mental illness surrounding there height - this is something I've struggled to connect with myself (I'm not saying others don't have some form of this) and feel much more aligned with the self improvement / improve quality of life approach
• Getting back to work / normal life quality and fully utilizing the weight bearing technology
• His calculated, almost "assembly line" type approach to the surgery - it seems like he has an amazing process down that he has now mastered
• How he views the procedure and its future

If I was getting the procedure done in the states I would definitely be considering Dr. Debiparshad. Thanks for putting this content out.

It's really great to see the passion these doctors have for their work, Dr. Gioktas seemed so happy when discussing LL and his patients.

I liked hearing him talk about the different generations of LL and the responsibility he feels to push the field forward in the right way, it will be really great to read his study when it comes out next year.

Data on funtionality, range of motion, recovery based on patients ratios and amount lengthening will be huge imo. These considerations are super important and will really help both doctor and patients decide on the best approach and goals.

Thanks! Really appreciate what you are doing! I was particularly interested when he talked about the fact that soft tissues do grow, not just stretch. As you are keeping your eye on the current research being done, do you think developments are more likely to be in either

a.Slowing the rate of bone growth so that soft tissues can grow in unison ie. comparable to puberty, or
b.Helping resistant soft tissues grow and adapt better

I am keeping my eye on research and although some ideas are really intriguing on the LL front, I know a lot of projects need funding amidst the economic impact the pandemic has had.

a.) However I don't know of any way to slow bone healing besides age, deficiency in nutrition and introducing harmful risk factors (smoking, too much anti-inflammatory affecting prostaglandins, and various other medicines and diseases). Hence the importance of x-ray check-ups to monitor the patients rate of bone healing and make sure it matches the patient's muscle flexibility.

b.) In my opinion the best way to do this is going to be through conditioning leading up to your procedure. I believe if you get manual therapy such as deep tissue massages, foam rolling for myofascial release, or a percussion massage gun for breaking up scar tissue adhesions which can really affect muscle rigidity during LL

I had asked my surgeon of he would be down for doing an interview like these, but he declined at the time. I wonder if he's changed his mind since then. A lot of what he said in my consultation mirrored what Dr Paley said in his video interview, particularly about fibula fixation and frustration with the doctors who try to get cosmetic patients in and out like an assembly line.

I think it'd  be cool if you try reaching out to Dr Parihar and link him to the Paley interview to see if he'd be cool with doing one now, especially since India is still a sought after budget-friendly alternative.
