Refracture after nail removal

Why is a stress fracture caused? I understand that if bone is healed enough, It shouldnt rebreak just like in the case of programa, how can It be avoided? Is It common or uncommon?

Its the only case i saw. Its very uncommon to happen, because the new bone is stronger than previous one. I hope programdude tell us what Paley said about that case, and how it happened.
