LL surgury pre requierties.

HI all,

I am new to the forum and i like to know what are the prerequisites for the LL surgery and for the surgery do some one have to sign as a Witness ex - Parent / Siblings / spouse

Different doctors have different requirements you have to meet before they'll operate on you.  Some make you write letters about why you feel you need the surgery and/or make you do a bunch of pre-op training, while others will just take you straight into the operating room.
