This question is a little silly. He is just too famous to do CLL. There is no way he can keep it in secret, even for 2 inches. The human society is just so brutal to dig out all the information to blemish anyone. People always laugh at or criticize those who did any plastic surgery even for small ones like double eye lids, nose, or lips or whatever. If people find out Tom did CLL, his career will end.
Why do people care about whether other did any plastic surgery? There would be many potential reasons. Let me think.
1. If other people look better by surgery, they will feel insecure because they will look worse or not better.
2. Those who wish to do surgeries but can't afford surgery because of money, time, or whatever reasons, they are jealous of people who look better after surgery.
3. Those who want to do surgeries but have no gut to do, they are also jealous.
4. It is nature of competition. All human wish to be better than others, So if they feel they are losing the game, they want to win the game by making fun of others.
Anyway, there will be more reasons. But, I just feel so bad about this kind of human nature. I think all people have right to live the way they want, if it doesn't affect others life. If ones want to do any plastic surgery, I believe that they should have right to go for it. Why others need to care about this? My biggest concern is also this on top of time and recovery. Anyway, whoever notices or makes fun of me in the future after CLL, I will just cut that relationship for good. That is all. I will keep or make friends who understand or do not know CLL.
