Author Topic: Lengthening Tibia 8CM vs Tibia and femur each 4CM at the same time?  (Read 2198 times)

Hey guys lets say cost wasn't included in this equation. Wouldn't lengthening the Femur and tibia 4cm each at the same time be better then lengthening just the Tibia 7.5CM? reason being is  there is less stretching of tissue/bone in that one segment and also for proportional reasons as well. I would think you would give a better recovery even though may take longer as the damage won't be as big but the pain during the stretching may be worse. What are your opinions? I remember reading Paley does allow bilateral lengthening.

Don't do bilateral lengthening .. the chance of fat embolism is too high to risk (it can lead to death). Tibial lengthening is best done externally as you 'may' get permanent knee discomfort through the manipulation of the tendon at the patella (knee cap). 7.5 cm is extreme in tibias. I am looking to lengthen up to 15cm and in my proportion calculations I have 7cm to achieve. Just because it is extreme you shouldn't be put off it. Just prepare yourself for the implications.

tldr: 7.5cm on tibias is not that smart if you want to lengthen 8cm. If you want to lengthen 8cm then do internal femurs first for 5cm and then 3cm on tibias using an external method. Internals aren't the end of the world. But the way they put the nail in can be problematic. However if it was a choice to do internal tibias with Paley or do external tibias with Sarin ... well I think everyone will agree that Paley is best ;)

$150,000 for bi lateral surgery on femur and tibas 5cm each max i would not do 4cm don't do 8cm on one segment.
