Hi guys, just saw Dr Debiparshad's post on instagram. Apparently he's offering full weight bearing option right after the lengthening phase with precice 2.2. which means 80 days after the implant it'll be removed.

Any idea about this?

You get precice 2.2 , after distraction he takes out the precice nail and puts in a normal non-lenghtening nail that you can weight bear on .
It's so many surgeries because then you have to remove that nail too and that comes down to 3 surgeries and more costs/risks .
Just someone like him can advertise such an option !

I have had that question a lot in the past few days.


The trauma littérature is still unclear if trauma nails are really weight bearing or not…

Exchange nailing is indeed an option, however it is an added surgery, one more passage of a nail through the gluteal muscles (more tissue trauma) Added risk of PE/fat embolism, extra cost. In my opinion it may at best speed up weight bearing by perhaps 30days, if at all.

A lot of my patients start partial weight bearing with a walker at the end of lengthening.

once again. exchange nailing at the end of the lengthening phase is an option, but i think may be better suited for delayed unions.

One also needs to accept the risk of trauma nail or locking screw  breakage if they decide to bear weight on an immature regenerate.
