How to measure tibiae and femurs?

There's a lot of debate about tibia/femur ratio on this forum but I cannot find any information on how you actually measure them.

Is it head of femur to medial condyle or greater trochanter to medial condyle?

Do you start from articular surface or medial malleous?

Also, does anyone have any link to medical studies and proper stats? I have seen some ratio posts on this forum but can't find anything that is backed up by proper results.
