Doctors with special conditions for doing surgery on you

What are the doctors who have special conditions for agreeing to perform leg lengthening surgery on a person? I don't mean just general lengthening limits I mean unique things you have to agree to before surgery, like for example Dr Guichet making you do a whole training program before. Do any other doctors have something unique they require before doing their surgery on you that you don't have to do with most other doctors?

Guichet - training program requirement
Mitkovic - I think you need to see a psychologist first
