Best Payment Methods for LL

For those who have had LL particularly in overseas countries what is the best method of paying for LL where you don't get too hurt by exchange rate fees?

Assuming you are staying in the foreign country throughout lengthening did you use prepaid debit cards, direct bank transfer?

Safest payment method that a reputable hospital might accept is VISA debit card (they shouldn't charge extra as it's debit).

Call your bank to ask about fees and then again right before you pay to tip them off about the large payment.

If you have a trustworthy immediate family member like one of your parents, you can set up a joint account with that person before you leave and transfer the money for surgery into that. Then after your surgeon gives you the payment details and you are comfortable with proceeding with surgery, you can send the doctor's payment details to the person on your joint account and they can bring it with them to the bank when they sign to initiate the money transfer into your doctor's account.

In the less affluent countries you might have some luck trying to barter with goats or 15 year old virgins.
