In case of bone infection, can the Ilzarov device solve the problem?

I'd rather say illzarov devices can potientially cause bone infection.

For what it's worth, Dr. Kulesh swears by Ilizarov devices - he said its "uncomfortable and takes a long time for patient" but very supportive for bone & studied and researched method with years behind it.

Local infection is very common with Ilizarov externals and is it's main disadvantage (only disadvantage for some). Pins must be very carefully be kept clean with proper bandages.
But if, or when, infection happens, allmost allways it is solved by local or local plus oral antibiotics. In extreme very rare circunstances, if infection reaches the bone, pins may have to be removed, and only inserted back after full infection resolution.

I'm talking about a condition where there is already an infection in the bones, in this case if you have an internal nail the infection will not go away until the nail is removed.

Not necessarily (you can still try oral antibiotics) but probably (and therefore need of removal and additional surgery).
