Both Tibia and Femur at Once or Seperately
« on: August 14, 2021, 08:35:13 AM »

Hello all,

Does anyone know what is suggested by the experts and science when it comes to deciding to do legenthing on the femur and tiba simulatiously or Seperately?

For example, of course the former offers a shorter time debilitated but may be more risky and painful?

Bilateral (separately) is offered by every surgeon and institute. Quadrilateral (simultaneously) is not offered in all locations. Usually what is suggested, and often enforced by experts offering quadrilateral, is smaller lengthening targets on each segment. I haven't seen any evidence anywhere regarding increased risk due to quadrilateral lengthening, but you can see from patient diaries and hear stories from patients at the clinics that pain, mobility, and overall well being is significantly worse doing quad compared to bilateral. The general consensus on the forum seems to be that you should shoot for max femurs, and return a year later if you really feel the need for more. On the other hand, if you care about lower/upper leg proportion and are SURE that you will lengthen under 12cm, then quad may be an option for you if you are constrained by time.

Not at the same time you increase the risk of embolisms and other complications which may lead to death.

The earliest account I have come across was Femur day 1 and Tibia 3 days after with the precise 2 nail. (Bilateral)

By quads I think he's talking about the staged tibia first, femur after 3 weeks. Anything less than that and you're asking for increased risk.
