Define safe. Normally the upper limits are 6cm(kinda pushing it). But with any LL the complications are sometimes a dice roll. You never know what you might get. Most of the time you get to walk and return back to normal daily activities minus strenous physical exercises. However chronic pain is an unknown factor as many people who had LL had some form of chronic pain or need further treatment for complications.

3-4cm is considered safe as in, most will achieve this without equinus contracture/negative dorsiflexion.

5cm is the top end for 90% of patients.

6cm+ less than 1% of patients will achieve (verifiable by diaries and speaking with real patients during consults).

Keep in mind this is for intramedullary nailing aka Stryde and will be completely different for external methods.
