Dr Parihar's private hospital may not look the best, but he performs many surgeries there and I'm sure he would use a good degree of sterilization in his OT to minimize the chance of infection. If it really bothers you, Dr Parihar will also perform surgeries at a separate hospital in Mumbai, but that option costs $5,000 extra in professional fees last I checked.
You should figure though that you will only be in the hospital for about a week, the rest of the time you would stay in a hotel. It's more important that your surgery be done correctly than how nice the hospital looks. Dr Parihar is considered to be the top expert in Ilizarov surgeries in India and was even recommended by Dr Paley when my friend in India asked him where he should go for CLL.
I'll take a hospital that doesn't look all that great if it means I won't have a screw popping out of my leg post op like what happened to Russianblues after returning from Sringari.
Of course if you did decide on Parihar you would likely need more than $20k anyway if you have bills back home you still have to pay for while in India. $20k would be cutting it really close.
